To apply for membership of Shared Care Scotland and gain access to all the associated benefits simply download our Contact/Membership application form and return it, marked for the attention of Lesley Gudgeon, to our offices in Dunfermline. Currently there is no charge for standard membership. If you'd rather not become a member but would like to be added to our contact list then select this option on the form.
