To apply for membership of Shared Care Scotland and gain access to all the associated benefits simply download our application form and return it, marked for the attention of Lesley Gudgeon, to our offices in Dunfermline. Currently there is no charge for standard membership. If you'd rather not become a member but would like to be added to our contact list then select this option on the form.
The application form can be completed on screen but you will then need to save the document and either return by email as an attachment or print and post.
